Completed PP, NC or HY training; current applicable CPR/first aid and documented scope readiness.
How learning works
Blended specialty cohort. Suggested pace: 3-4 weeks plus completion of the 4 verified practice hours.
Practice & application
Four verified hours on one approved transition case, including at least two directly observed education/handoff hours and two supervised coordination/debrief hours. Complete four module artifacts, an evidence comparison and reflective summary; pass the knowledge quiz at 34/40 and integrated case at 17/20 with every dimension at least 3 and no critical error.

Scope & credential clarity
An advanced specialty certificate, separate from principal certification. Clinical decisions remain with the licensed care team.

Module 01SP101 Discharge and continuity
Create a transition plan that reflects the written discharge instructions and family priorities.
300 minutes instruction + 90 minutes independent study.
Topics: Care-team roles; access; consent; discharge questions; feeding and recovery continuity.
Assignment: Draft a discharge-to-home support map and escalation directory.
Module 02SP102 Multiples and complex logistics
Organize nonclinical support around multiple caregivers and complex routines.
300 minutes instruction + 90 minutes independent study.
Topics: Multiples; equipment logistics; appointments; transport; shared records; fatigue.
Assignment: Build a household shift and appointment plan from a de-identified case.
Module 03SP103 Recovery and emotional adjustment
Respond to recovery and emotional concerns within scope.
300 minutes instruction + 90 minutes independent study.
Topics: Parent recovery; grief; isolation; inclusive communication; warning signs and referral.
Assignment: Complete a support-and-referral case and compare two evidence sources.
Module 04SP104 Case conference and service handoff
Coordinate a consented handoff and document the limits of the support plan.
300 minutes instruction + 90 minutes independent study.
Topics: Team case conference; handoff; offboarding; escalation; reflective review.
Assignment: Submit a transition portfolio and case presentation tied to verified practice.
Final assignment
Present a coordinated transition case and the four module artifacts, evidence comparison, reflection and signed practice record.
Materials and preparation
Digital course workbook, scenario pack and assignment templates; access to a device for reading and submitted work. Use fictional or de-identified examples. Faculty assign selected readings within the stated study hours.
